Role Summary

- We are seeking a highly organized, fast-paced coordinator to manage incoming referrals and
- match families with the right providers. This is not a data entry role — it requires strong
- communication, decision-making, and follow-through to ensure services are scheduled quickly
and efficiently.
Responsibilities
- Respond to all new referrals within 30 minutes of receipt
- Match families with appropriate providers based on language, schedule, and location
- Coordinate scheduling between families and providers until services begin
- Maintain consistent follow-up and communication with all parties
- Track referral status, outcomes, and reasons for lost referrals
- Report recurring issues or service gaps to the team
- Maintain accurate documentation for reporting and audit readiness
Requirements
- Fluent in English and Spanish (written and verbal)
- Strong communication and follow-up skills
- Highly organized with the ability to manage multiple tasks at once
- Sense of urgency and ability to work in a fast-paced environment
- Comfortable using systems to track data and manage workflows
- Strong problem-solving skills and ability to make decisions independently
